Faculty Evaluations / Tenure Review Forms

To access all forms for evaluations of any faculty — tenure track, tenured, or part-timer — use the links on the top/left bulleted list and the last entry on the thin gold bar above.   The forms are all Word documents that can be downloaded, saved on your computer, then completed and printed out (or printed out and completed by hand).  DO NOT MAKE ANY CHANGES TO ANY OF THE FORMS or they may be thrown out and won’t count for the evaluation being done.

Most of the forms are found under the TR Forms/Tenure Track or Tenured Faculty Evaluation Forms link, with those that change annually (based on a calendar) being grouped separately.  Consult your current Faculty Evaluation Policy & Procedures Handbook for all procedural information on evaluations of tenure track or contract faculty.

The new evaluation forms for Part-Time Faculty pursuant to Article 30H of the PFT-PCCD Contract (eff. 3/22/06) are also accessed through the respective links to the part-time faculty evaluation forms that exist to date:  i.e., those for all part-time classroom faculty and those for part-time counselors.   As new forms for other part-time non-classroom faculty are developed and approved, they will be added accordingly.
