2014-2015 Financial Aid Forms
If you were directed to this page, Your FAFSA has been selected for a review process called verification. We are required to request copies of certain financial documents from you, your parents, or your spouse in order to process your financial aid file.
In addition to the verification form below, you, your spouse, and/or your parent may be required to submit a 2013 tax transcript into the financial aid office. The tax transcript is required if you are interested in receiving financial aid for the Fall 2014, Spring 2015, or Summer 2015 term. If you, your spouse, and/or your parent do not have a copy of the tax transcript, he/she may order a Tax Return transcript from the Internal Revenue Service online at www.irs.gov. Click on the “Order a Return or Account Transcript” link, and click on “Order a Transcript” under number 3. Enter your personal information and select continue. Under the “Type of Transcript” option, please select “Return Transcript”. Also select 2013 as the year. You may also call 1-800-908-9946 to request this information. Please note copies of 2013 tax returns are not acceptable documentation to fulfill this requirement.

2014-2015 Board of Governors Fee Waiver
A Board of Governor’s Fee Waiver (BOGFW) is a tuition waiver. This waiver is only applicable to tuition fees and will not apply to a student’s campus
fees, health fees, AC Transit Bus pass or fee based courses. Since this is a waiver there is no actual disbursement of funds. The Board of Governor’s Fee
Waiver is only available to California Residents and AB540 students.
